How to Request a Custom Network to be added to Custom Network List

Description
With the new ‘Custom Network List’ experimental feature for MetaMask, one is directed to the ‘Feature Request’ forum from the ‘Can’t find a setting? Request Here’ link.
When I tried to request a Network to be added, I referred to the name of said network as that seemed intuitive to me, but according to the forum rules one are not allowed to do that.
In hindsight, telling the ChainID might be enough, but I’d like clarification on this for proper filing or a template.

What is the proper procedure or template for requesting a ‘Custom Network’ to be added to the new experimental ‘Custom Network List’?
