Create Account resulted in duplicate account

I attempted to create a new account. I previously had three existing ones. After getting prompted for the account name a fourth account was created but it was a duplicate of one of my previous ones. The name, address, balance, etc. are all the same. The new account and name I wanted wasn’t created.

  1. How can I delete the duplicate account without also removing its twin?
  2. How can I create a new account and avoid this duplication?

I am using latest Brave Version 1.29.76 Chromium: 93.0.4577.58 (Official Build) (64-bit) on Windows 10.
